The torturer prepares to flay the St. Bartholomew with a knife. Conceivably the rope tying the saint's left hand is an indication that, as the Golden Legends suggests, he was first crucified, then taken down before he expired, and then flayed.

It is not clear what the hand in the lower left corner is all about.
